The equipment

Antares is a piece of equipment microcontrolled by the photobiomodulation therapy, and was developed for use in different areas of health and aesthetics. It presents the possibility of operation with single applicators (probes) and applicators with groups of laser and/or LED (cluster), which offers a new dimension of light therapy.

The Antares treatment is not thermal, invasive or ablative. The irradiation of low energy photons (phototherapy) is generated by low intensity laser, (LLLT) – “Low Level Laser Therapy” –, or by LEDT (Light Emitting Diode Therapy), which can modulate several biological processes. This phenomenon is known as photobiomodulation.

Presenting different configuration and technical characteristics, Antares offers a wide choice of selection of wavelengths (wavelength of light or combinations between laser diode and/or LED diode) and can be used to treat several conditions and pathologies.

The equipment must be used only under prescription and supervision of a licensed professional.

Photobiomodulation

A number of descriptions have been used to define the therapy by low intensity light, such as: photostimulation, phototherapy, lasertherapy, led therapy, cold laser, soft laser, among others.

Photobiomodulation, therefore, describes the therapeutic use of visible light and infrared radiation absorbed by endogenous chromophores, unleashing bioactive reactions, non-thermal, non-cytotoxic, through photochemical events.

When referring to light therapy, it is important to highlight that the therapeutic effects may occur both in the form of stimulation (increase in the production of ATP, NO and ROS and increase in the liberation of endorphins) and inhibition (decrease of inflammatory substances, such as interleukins, prostaglandins, etc.). Therefore, the description “photobiomodulation” sounds more adequate.  

Wavelength (nm)

Wavelengths are divided into two main groups: bactericides/fungicides and regenerators. The violet-blue range presents bactericidal/fungicidal effects by liberation of reactive oxygen species which destroy fungi and bacteria. The range between 400-420nm presents greater antibacterial effect (violet-blue), while the range between 420-470nm (deep blue) presents less antibacterial effect. Recent research suggests that the wavelength of 450nm presents hydrating effects.

It is important to point out that the indiscriminate use of violet-blue light can increase the oxidative stress of cells in the dermis and inhibit the activity of fibroblasts.  This effect is dose-dependent. Therefore, in certain situations, the use of regenerating wavelengths is indicated after violet and/or blue irradiation.

Wavelengths in the spectral range which favors healing and tissue regeneration are green, amber, red and infrared. The penetration of light in the tissues varies according to the wavelength (nm) and its approximate penetration can be observed in Figure 1.

One of the main objectives of photobiomodulation applied to aesthetics is to reach as many fibroblasts as possible. The amber range, for instance, causes light to reach the papillary dermis and the red, the reticular dermis. The association of these two wavelengths have been very well-accepted in treatments which promote skin rejuvenation.

The therapeutic effects are also dependent on the dose applied. For example, a regenerative wavelength can become inhibiting if the dose is considered high. In extremely high doses, the photothermal effect is very significant, and they are used, for example, in intense pulsed light selective photothermolysis and in high power lasers, for aesthetics purposes.

The Arndt-Schultz law clearly demonstrates this and describes the “therapeutic window” (Figure 2). Some authors compare the effects of “therapy by light” with the effects of medications. In very low doses, they have no effectiveness; however, in very high doses, what was initially a medicine can become a poison. There is a therapeutic dosage, where the optimal effects are achieved. The same occurs with photobiomodulation.

Mechanisms of action

Photons are discrete particles of pure light energy. The first law of photobiology affirms that without absorption there cannot be reaction. So, the incident photon must be absorbed by the target-cell so that any reaction occurs. When a photon is absorbed by a cell, it transmits its energy to the cell.

Chromophores are natural photoreceptors in our bodies. The endogenous pigments melanin and hemoglobin are the main chromophores excited when the desired objective is the selective photothermolysis, and they are heated to high temperatures during the treatments, as in photoepilation and sclerosis telangiectasia. In photobiomodulation, wavelengths in the violet-blue range are strongly absorbed by melanin, therefore, the client feels more local heat, particularly the ones with higher phototypes.  

Among the non-pigmented chromophores, the enzyme cytochrome C oxidase, which is located in the respiratory chain of the mitochondria, is the main photoreceptor of photobiomodulation.  The cytochrome C oxidase presents two peaks of absorption, in the red and near-infrared spectra. Photobiomodulation seems to increase the availability of electrons to reduce molecular oxygen, increasing the levels of ATP, NO and ROS, energizing the cell and favoring cellular activity.

Studies also demonstrated that photobiomodulation stimulates the release of growth factors, such as:  VEGF, FGF- and KGF, all related to repair processes and tissue regeneration.

Besides, it is well-established that this therapeutic modality activates factors of gene transcription, exerts antiapoptotic and anti-inflammatory effects and decreases cellular oxidative stress, as well as stimulating the proliferation, migration and synthesis of proteins related with the maturation of epithelial cells.

Safety

In this type of treatment, the use of protection goggles is mandatory for patients, operators, maintenance personnel and any person in the environment while the equipment is in operation.

Competitive Differentials


Antares is electromedical equipment microcontrolled by phototherapy (biophotonic therapy) which was designed for use in the areas of aesthetics, podology, dermatology, physical rehabilitation and laser puncture.  It allows the change of applicators, which can be a probe or a cluster, with different configurations and technical characteristics. 

 

Light Emission Check

Antares presents a verification system so that the of LED/laser emission of the applicator can be validated before use. When the equipment is turned on, or the applicator is reconnected or substituted, it will be necessary to check the emission only once before use.

 

Presets

The presets present in Antares direct the ideal therapeutic use. Adjustments in dose can be made by the therapist according to clinical needs. The Arndt-Schulz Law affirms that there is but a narrow therapeutic window in which one can really activate cellular response using precise sets of parameters; that is, fluence or dose.

This equipment allows treatments using different sequential wavelengths: 2C (bichromatic – two wavelengths) or 3C (trichromatic – three wavelengths).

Positioning of probes and clusters

Precautions

Preparing the patient for therapy

  • The patient must be positioned comfortably according to the instructions (sitting or lying position);
  • A minimum distance of 20cm must be respected between operator and patient during the treatment;
  • Examine the skin and clean the treatment area with water, neutral soap and alcoholic chlorhexidine at 0.5% or rubbing alcohol 70%;
  • After cleaning the skin, it is necessary to wait until the product evaporates, not posing risk to the patient;
  • Clean the probe or the cluster before and after each session, according to the instructions about cleansing of the applicators described in these instructions of use;
  • It is also important, after each application, to clean the applicator lenses with a cotton ball humidified in alcoholic chlorhexidine at 0.5% or rubbing alcohol 70%;
  • Examine the skin again after the treatment;
  • The correct preparation of the skin for the light therapy enables more energy of the light beam to reach specific target-areas e reduces the risk of skin irritation.
